Insured Health & Hearing Services-Hormone Therapies for Gender Affirming Care Policy is available (Policy DP17)

• To access the Benefits under this policy a medical practitioner must apply to the Chronic Disease and Disability Benefits program on behalf of the patient

• Criteria for Gender Affirming Procedures

  Must have a valid Yukon Health Care Insurance Plan number,

  Well documented gender dysphoria and specific dysphoria related to the particular intervention as determined by one qualified assessor,

  Capacity to make informed decisions and give consent for treatment,

  Persons with guardians or other decision makers are managed with the inclusion of all parties involved in decision making.
